This form is now an adobe sign document, but does not allow for the authorized rep. to sign or fill in any information. Are they accepting this form with just the member listing the ARD’s name and contact info?
Error on application PDF’s
Good morning! On both the SACA application, the DOB section says to format it (MM/DD/YYYY) however, the year only allows for 2 characters and cuts off the last two if you type out the 4 digits. Not sure who to direct this to!

If a client has a private POS plan with out of network benefits, will MH secondary pick up the co-pays, co-insurance etc. for an out of network provider?
